Who is this for

CareLink360 is the only comprehensive forms and document management platform for all at home health, care and comfort providers. We classify “at home health, care and comfort providers” into four groups:

Home Health

Home health providers provide clinical medical care. The service is provided by a registered nurse, occupational therapist, physical therapist or other skilled medical professionals. Home health is often prescribed as part of a care plan following a hospitalization.

Home Care

Home care providers primarily supply home care aides. Home care aides can help older adults with activities of daily living, or just offer some friendly companionship. Unlike home health, home care is classified as personal care or companion care and is considered "unskilled" or "non-clinical."

Hospice Care

Hospice providers are focused on improving quality of life instead of prolonging treatment. Hospice care is usually offered only when the person is expected to live 6 months or less. Services include medical services, emotional support, spiritual services etc.

Palliative care

Palliative care organizations provide palliative care for people with serious illness. The support staff here includes doctors, nurses, social workers etc. who help with improving quality of life.
